DO YOU KNOW WHAT A BREATH-CUTTING SPAN IS? Breath-cutting spasms are classified as a type of non-epileptic paroxysmal disorder, which are episodes that mimic an epileptic seizure, although they are not actually seizures. They generally appear suddenly and last for a short time. We don't fully understand the cause, but we do know that they are not due to an excessive neuronal discharge, as is the case with epilepsy. HOW DO BREATH-CUTTING SPAS MANIFEST? They occur in children between six months and five years old who, after a bump or fall, a scolding, a tantrum, or a strong emotional outburst, become pale and stop breathing for a few seconds. They may turn blue until, after a few seconds, they start crying again. If the episode lasts longer than 10-15 seconds, some jerking movements, similar to convulsions, may occur. WHAT CAN TRIGGER A BREATH-CUTTING SPASM? The trigger can be a fright or a sudden shock, or pain after a fall or a bruise, although the most typical cause is a spasm triggered by frustration when the child is denied what they want, in the context of a tantrum or intense crying of any origin. WHAT SHOULD I DO DURING THE EPISODE? Stay calm. These episodes generate a very high level of anxiety and worry in parents. The less than ten seconds that the cessation of breathing usually lasts feel like an eternity to them. But the truth is that we can't do anything, and it's also important to remember that we shouldn't shake or hit the child to bring them back to consciousness, nor should we perform resuscitation maneuvers, as we could even harm or injure the child. Pediatricians should explain to parents that the episode will pass and the child will return to normal, that they shouldn't do anything except remain calm and observe the child, however difficult it may be for us not to do anything. WHAT IS THE TREATMENT? Breath-holding spells, like other neurological disorders of speech and language (NDS), have no treatment. They improve with age and neurological maturity in children and will eventually disappear over time. There is also no preventative treatment; do not administer relaxants, herbal teas, or similar remedies without consulting a pediatrician. Currently, there is no medication indicated for children with breath-holding spells. CAN IT LEAD TO EPILEPSY? No. Children recover completely after each episode, and there is no association with any disease, including epilepsy. However, if the parents experienced breath-holding spells in childhood, their child is more likely to experience them as well. SO, CAN I NOT DENY MY CHILD ANYTHING OR DISAGREE WITH THEM? This is one of the main questions parents ask when we explain the diagnosis. It's understandable; if we tell them that frustration or tantrums can trigger an episode, parents will usually try to simply prevent the child from getting angry. However, it's important to establish the same limits and guidelines as with other children their age. We should try to avoid situations that cause fear or strong emotions, but remember that breath-holding spells will disappear with time, whereas if we've taught the child that they can get everything as soon as they ask for it, this behavior will be more difficult to correct later on.
