Can Spine Surgery Cause Paralysis? முதுகெலும்பு அறுவை சிகிச்சை பக்கவாதத்தை ஏற்படுத்துமா? Hello! I am Dr.Vijayaraghavan. I am a spine surgeon. Normally when we advise spine surgery for a patient, a common question that they all ask is "does spine surgery result in paralysis?" That is, is there a chance for hands and legs to become immobile? What is the probability of paralysis? This is a common question. This video is a small effort to provide answers to that question. Before we get to the explanation first, how does our nervous system and muscles function? Let me give you a brief description of the same. In simple terms without using many medical jargons. Our spinal cord originates from the brain and travels through a small tunnel in our spine bones, similar to a train that goes into a tunnel. The spinal cord ends at the mid back. Below that, nerves branch out from the spinal cord. That is only called ‘Cauda equina’. So the part where the spinal cord ends, it is called L1 in medical terms. Apart from this, the spinal canal through the small holes on either side, nerves branch out from the spinal cord. The spine nerves travel through the muscles and provide nerve supply to various parts. If you notice, this is like a small electrical circuit in total. The main power supply comes from the brain through the spinal cord, like a transformer cable. So these spinal nerves are like wires. Similar to how the wires provide electrical supply through the walls, the same way, nerves traverse through the muscles and provide nerve supply. Coming to the question, When does paralysis happen? When does the power supply completely stop? If there is a problem with the transformer or the main source of power, the power supply will be affected. Likewise, if there is an injury in the spinal cord there are chances for paralysis. If the spinal cord is injured in your neck region, hands and legs can become paralyzed. If the spinal cord is injured in the upper back region legs can become immobile. This is what we call paralysis. In the lower back, like I mentioned earlier it is not the spinal cord. There is only the nervous system. The chances of paralysis in that area is almost nil. If you consider the most common spine surgeries are done in your lower back, which is called the lumbar spine. It can be slip disc, Spondylolisthesis, the surgery done to correct the abnormal movements in between the lumbar bones, similarly, lumbar canal stenosis, a surgery done to release the nerve compression in the lower back. If you look at the symptoms, sciatica, all the surgeries done to rectify the numbness in the leg, most of it is done in the lumbar spine. Of the surgery done in the lumbar spine the risk of paralysis is almost nil because you do not have a spinal cord there. The main power supply is not there. You just have the spinal nerves. So the message is, if your spine surgeon has advised and if the spine surgery is advised in the lower back or the lumbar spine, the chance of paralysis there is very less. So coming back, is there the risk of paralysis in a surgery that is done in the upper back and the neck region, if that is your question the risk in that too in common surgeries is less than .5%. With the new technologies like neuromonitoring, navigation and when complex spine surgeries are done using these technologies the risk of paralysis is very less. What is neuromonitoring?
