Enclose the $5.00 fee in form of check with the filled out application. Mail it to the address at the top left of the paper Eventually, they will mail you back an assigned VIN plate and your paper. (You can avoid the wait if you visit dmv in person with the paper filled out) Install VIN plate as per instructions on paper. Call your local state police department and they will send out a trooper to inspect the trailer VIN plate installation. He will sign the paper and return it to you.
