The documenting of a record for a new patient added to a Cancer Registry requires hundreds of data points.  This patient data is critical for the ongoing care of the patient and can take hours to obtain from numerous providers of care.  The improving state of interoperability across the nation has made this critical patient data much easier to obtain.  This webinar, talking directly with Ohio Cancer Centers, will share their efforts to utilize CliniSync’s Community Health Record (CHR) to improve the quality & efficiency of patient data obtained. Information in the HIE includes records from the VA and the Department of Defense. The Centers will also share a guide for the CliniSync CHR that they helped compile that will show how data can be located more quickly within the longitudinal health record. This guide can also assist future treating providers in finding treatment data normally provided through a Survivorship Care Plan. Join us for a wrap-up round table with the Cancer Centers on using these tools to enhance survivorship care planning.
