NSAIDs inhibit prostaglandin synthesis by antagonizing cyclooxygenase. Normally, prostaglandins lead to vasodilation of the afferent arterioles and vasoconstriction of the efferent arterioles in the kidneys (resulting in high GFR). If this effect is inhibited by NSAIDs, GFR drops and sodium and water retention increase, resulting in increased blood pressure.
