In this video, I talk about varicocele, which are dilated, varicose veins in the testicle, addressing cases where it is related to pain. I emphasize that varicocele rarely causes pain and that each case should be thoroughly investigated, characterizing the pain, which is usually a dull, uncomfortable, prolonged pain that sometimes worsens when the individual stands or exerts themselves. It is important to perform a physical examination to quantify the degree of varicocele and identify other possible causes of pain. Ultrasound can also assist in the diagnosis, and other causes of pain such as inguinal hernia, herniated disc, or renal colic that present with pain radiating to the testicle should also be ruled out. The treatment is initially clinical, with the use of analgesic medications and a scrotal support, but in cases where this approach has been used for a prolonged period, surgical treatment should be proposed, always informing the patient that surgery may not completely resolve the pain. The surgical techniques that show the best results are the microscopic inguinal or subinguinal techniques. Several characteristics can influence the outcome, such as the degree of varicocele (the higher the degree, the lower the chance of pain improvement), the type of pain (characteristic and with a longer duration, which tends to show better results with surgery), and patients with a high body mass index (BMI), who will have a worse response to surgical treatment.
