WEIGHT LOSS — Asbaghi 2020 — 12 RCTs, ~2 kg weight loss over 8-12 weeks. STEP 1 trial (Wilding 2021 NEJM) — semaglutide ~15 kg over 68 weeks. Berberine is 4-5 lbs over 12 weeks. Semaglutide is 30 lbs over 68 weeks. Same pathway family. Different magnitude. Not "nature's Ozempic." SAFETY — THE CONVERSATION NOBODY IS HAVING Berberine affects statins through two different pathways: • CYP3A4 inhibition → can raise blood levels of simvastatin, atorvastatin, lovastatin → muscle damage risk • OATP1B1 induction → can increase rosuvastatin liver uptake (Feng 2020 Frontiers Pharmacol) If you are on a statin and considering berberine — you cannot stack them without your prescribing physician knowing. Monitoring required. SUPPLEMENT QUALITY — The FDA does not test what is in the bottle before it reaches your cart. Pick a brand with NSF Certified or USP Verified marks on the label. Without those, you do not actually know what you are taking. MARK — 6 MONTHS, NO MEDICATION CHANGE Mark, 62, retired aerospace quality engineer, suburban Seattle. Outside those two cases — the bigger levers do more for less risk and zero supplement budget.
