Have you ever heard that pumpkin seeds are good for the prostate? Is it true or a myth? What does science really say about this? Benign prostatic hyperplasia, or BPH, is very common after age 50. Symptoms include: • Frequent urge to urinate • Getting up at night (nocturia) • Weak stream • Sensation of incomplete emptying Many patients seek natural alternatives to manage this before turning to medication or surgery—and that's where pumpkin seeds (Cucurbita pepo) can be of interest. Some scientific data suggests that pumpkin seeds can improve mild to moderate urinary symptoms with good tolerance and without adverse effects on sexual function. Mechanisms of action include: • Phytosterols, such as beta-sitosterol, which can inhibit the enzyme 5-alpha-reductase • Antioxidant and anti-inflammatory action • Inhibition of the proliferation of hyperplastic and cancer cells, as shown in vitro studies • Example: A study by Medjakovic et al. (2016) showed that pumpkin seed extract inhibited the growth of hyperplastic cells and prostate cancer cells independently of steroid hormone receptors. However, we need to be careful and critically analyze the studies already conducted on the subject: what are the limitations of these studies? 1. Few randomized controlled clinical trials: Most studies are observational or pilot studies, without a placebo group. This means we cannot say with certainty that the improvement was caused solely by the extract. 2. Subjective outcomes (IPSS): The IPSS is a good questionnaire, but it relies on patient self-reporting, which can be influenced by the placebo effect or expectations. 3. Short follow-up time for most patients. 4. Lack of extract standardization: Studies used different types of extracts: oil, dry powder, and water-soluble extracts. Without phytochemical standardization, it is difficult to replicate results and recommend specific doses. 5. Potential conflict of interest: Some studies were funded by extract-producing companies, which may introduce bias. In summary: • Pumpkin seeds have studies that show benefits, particularly in improving mild to moderate BPH symptoms. • It can be used as a complementary therapy, with a good safety profile. • However, we still don't have enough robust evidence to replace it with conventional pharmacological treatments in more advanced cases.
