I-485 AOS Folder: Cover Letter Payment of $1,225.00 for Filing Fee and Biometrics Fee in check or money order payable to "U.S. Department of Homeland Security" Form G-1145, E-Notification of Application/Petition Acceptance (optional but recommended so you will get updates of your case) Duly accomplished and signed Form I-485, Application to Register Permanent Residence Copy of I-797, Notice of Approval for the I-129F Petition (NOA2) Copy of NVC acceptance letter with the case number copy of marriage certificate Copy of birth certificate Copy of biographic page of passport Copy of K1 visa page Copy of Form I-94, Arrival-Departure Record Two (2) 2"X2" size colored photos with full name and A# at the back Copy of DS-3025, Vaccination Documentation Worksheet Proof of Relationship such as wedding pictures, health insurance, joint bank acct, proof of billing (optional) Duly accomplished and signed Form I-864, Affidavit of Support of the petitioner and supporting documents: Copy of Form 1040 recent year or recent 3 years (optional) Copy of W2or source of income documents recent year or recent 3 years (optional) Copy of IRS tax transcript (recent year) or recent 3 years (optional) Employment letter Recent three (3) months pay stubs Biographic page of passport of petitioner Copy of birth certificate of the petitioner If the petitioner's income does not meet the required Poverty Guidelines, you will need to find a co-sponsor who has enough income to support his/her family and dependents PLUS the immigrants. The co-sponsor will need to submit the following documents: Separate Form I-864 or Form I-864A (if co-sponsor resides in the same house as the petitioner) Copies of Form 1040 or IRS tax transcripts Copies of W2’s or source of income documents Copy of US passport or birth certificate (if co-sponsor is a US citizen) OR copy of green card (if co-sponsor is a permanent resident) Duly accomplished and signed Form I-944, Declaration of Self-Sufficiency of the AOS applicant and supporting documents: Proofs of relationship for all household members Proofs of income of all household members - income tax return, W2 or other proofs of income If any of the household member is receiving additional income, provide proofs of income Proofs of assets - property titles, most recent appraisal of property, bank account statements, investment documents, etc. Proofs of liabilities - credit card statements, student loans, car loans, personal loans, etc. Credit report (if any). Credit score (if any).
