Clomipramine, the most serotonergic (serotonin-augmenting) of the first-generation tricyclic "antidepressants" (TCAs), was the first agent approved by the FDA for the treatment of OCD (obsessive-compulsive disorder) and its use foreshadowed the SSRI era. It is a strong antidepressant, but also an excellent anxiolytic agent, and does well in combination with other psychotropic medications in low doses. Unfortunately, the TCAs are rife with side effects and are potentially lethal in overdose, and so must be used with caution. Clomipramine is still considered a first-line agent in the treatment of OCD.
