Open vs. endovascular aneurysm repair in the 60-year-old man against proposed NICE guidelines

“The best treatment for a 60-year-old man with an infrarenal abdominal aortic aneurysm that is above the threshold for intervention is arguably surgery based on the durability of this intervention. However, operating at this age (where open surgery is a good option) is unusual in the UK and the majority of patients who present with an aneurysm are older and less fit. This is the group that do benefit from EVAR as long as they are anatomically suitable. I do not agree with the UK’s proposed draft NICE guidelines because they do not make allowance for consideration of anatomy,” says Michael Jenkins (St Mary’s Hospital, London, UK) at the London Aortic Symposium 2018, where this topic was debated.
