In this special series on Contact Lenses, Dr. Tiago César teaches you how to remove scleral lenses and how to store them safely and hygienically. Always remember to properly wash your hands with soap and water, and if possible, dry them with paper towels to avoid lint from the towels. It's important that your hands are not wet. Dry them well! This also applies to cases and suction cups. Tap water can contain microorganisms capable of causing eye infections. Always start insertion with the same scleral lens and routinely remove the same scleral lens. This avoids having to replace scleral lenses. Starting with the eye with less vision makes the process easier. Using only the multipurpose solution specific to your lens is essential! Do not wash with tap water! Saline solution is not recommended for cleaning/storing! Do not use multipurpose solutions for other types of lenses! The lens-specific multipurpose solution is specifically designed for cleaning, rinsing, disinfecting, and moisturizing lenses. For scleral lenses, we use saline solution in vials to fill the inner, concave surface of the lens. The risk of using bottled saline solutions is the preservative content and the potential for contact lens contamination! Infectious ulcers in scleral contact lens wearers should be carefully avoided! After removing your lenses from their case, place them in the palm of your hand and gently rub all sides of the scleral lens. You can apply some pressure, but be careful not to be too vigorous and damage the scleral lens. Rinse the scleral lens with the specific solution recommended for you. After attaching the scleral lens to the suction cup, you can rinse the inner surface with saline solution in vials. This is an optional step. Use the tips of your first three fingers together to secure the scleral lens or use the scleral lens suction cup that comes with your kit. The scleral lens insertion process must be very careful: Dry your hands thoroughly, secure the lens with the concavity facing up. Fill the lens generously with saline solution in vials. The scleral lens must be completely filled with saline solution, level with the rim. If it's too full, it may leak too much saline; if it's a little empty, it may develop air bubbles or be too close to the eye (a bit tight). Position your head parallel to the plane of the lens (chin closer to your chest). With your free hand, hold the upper eyelids. With your fourth and fifth fingers (not holding the lens), expose the lower eyelid. The eyelid should be exposed enough for the lens to fit in without being touched or tilted. Once all the planes are aligned, fix your other eye on a point on the table (this can be a marker or even a mirror from your kit) and gently insert the scleral lenses into your eyes. No force is needed during this process. After inserting the scleral lenses, blink gently and dry your eyelids. Take a paper towel and dry the inside of your case, including the lid. Every two days, clean the case more thoroughly using water, mild soap, and a small brush. Dry the case. Apply a thin layer of multipurpose solution to the case to disinfect it and dry it again. Remember to also wash your suction cups with mild soap and water. They should also be dried, and before contact with the scleral contact lens, apply a thin layer of multipurpose solution again to disinfect the suction cup.
