Chaplain Documentation: Best Practices and Emerging Research

How are chaplains documenting their work in the medical record, and what should chaplains include in their notes? What do other medical providers identify as important and helpful in chaplain notes? Three chaplains who have completed research in the area of chaplain documentation will be presenting their findings.
