He himself said he was constantly throwing tantrums and breaking dishes." --------------------------------------------------------- Hebephrenic Form (F20.1) Hebephrenia (from the Greek "hebe" - youth) is a form of schizophrenia that develops during puberty. It was first described by the German psychiatrist E. Hecker in 1878. The disease begins gradually, with distinct signs of negative personality changes - weakening emotional attachments, a paucity of interests, autism, and impaired thinking. Psychological characteristics typical of puberty - the desire for independence and autonomy - are exaggerated and take on a grotesque character. A narrowing of interests and contacts is observed, along with a leveling of emotional reactions and a distorted nature, manifested by an unmotivated antipathy toward loved ones and "blunt hostility." Motor skills also change: the harmony of movements is lost, the gait takes on a strange, "walking on stilts" quality, intonation and gestures lose their coherence with the content of speech and the situation; facial expressions become inappropriate, the voice unnatural, and stereotypical, pretentious movements may occur. Behavior is characterized by crude mannerisms, silliness, grimacing, caricatured childish (puerile) behavior, the commission of absurd acts, cruelty towards family members, and an empty, unproductive euphoria (stupid gaiety). This is accompanied by drive disorders (like impulsive, irresistible urges) - vagrancy, alcohol and drug abuse, gluttony, and sexual disinhibition. Hebephrenic agitation is characterized by aimless motor activity and impulsive, aggressive actions. Patients appear detached from their surroundings and may not respond to changes in the environment or to being addressed. Speech becomes exalted and pathetic, words are distorted, and pronounced in an unnatural voice. Patients jump, tumble, roll around on the floor, pester others, swear obscenely, commit aggressive acts toward others, or, conversely, coo and try to kiss. Sexual disinhibition and uncleanliness of urine and feces are observed. The peculiarity of hebephrenic arousal, according to A.E. Lichko (1989), is determined precisely by the "splitting of affect," when cruel aggression is accompanied by laughter, and a frozen smile is combined with a fearful gaze; cursing is accompanied by a desire to kiss, and sweet words are accompanied by the intent to hit. The hebephrenic form of schizophrenia is considered a fleeting, malignant variant of the disease. Dementia (defect) develops quite rapidly—within 1-2 years—with symptoms of persistent, treatment-resistant hebephrenic syndrome, lasting for at least half a lifetime. Along with apathetic-abulic disorders and emotional dullness, mental retardation is observed at the stage when the disease manifests. We present a clinical example from forensic psychiatric practice. Another patient, P., 38, was accused of corrupting underage girls. P. lived alone in a room, and at night, according to neighbors, he danced, sang, and laughed. One day, he ran out into the street in his underwear, jumped on a passing tram, and was taken to a psychiatric hospital. Over the past ten years, he had been hospitalized numerous times. There, he was noted to have a fragmented mental state, untidiness, mannerisms, and emotional devastation. During the examination, the patient made ridiculous jokes (silliness), unexpectedly blew in the faces of those around him, grimaced, and silently whispered something, claimed to like the taste of urine, and tried to drink it. His condition sometimes changed unexpectedly: he became cynical, spat, mimicked everyone, or laughed until he cried, sticking out his tongue and shouting nonsensical phrases.
