In this video, Dr. Jean discusses the possibility of bladder changes after hysterectomy, the surgery to remove the uterus, a common question in his office. Many patients wonder if urinary function will be compromised or if there is a risk of incontinence after the procedure. In most hysterectomies, especially those to treat fibroids, adenomyosis, abnormal bleeding, and some forms of endometriosis (where bladder intervention is not necessary), the bladder's supporting structure is not affected. The bladder remains supported by the pelvic structure, which includes muscles and fascia. There is a natural support in the front of the vagina that protects the bladder, while in the back, there is a structure that supports the bowel. In hysterectomies for benign pathologies, the uterus is removed and the fundus of the vagina is sutured, leaving the cervical ring and pelvic structure intact. Therefore, these supporting structures are preserved, without any additional risk of bladder prolapse. However, it is worth noting that factors such as aging, collagen reduction, and menopause naturally increase the risk of bladder prolapse in women between 40 and 55 years of age. Other factors, such as natural childbirth and high-impact exercise, can also weaken the supporting structures. In surgeries to treat uterine cancer, on the other hand, it is common to completely remove the cervical support network, increasing the risk of prolapse. On the other hand, radiation therapy, often indicated as a complementary treatment, can firm the area, reducing the risk of prolapse in the future. The risk of bladder prolapse after hysterectomy is higher in women who already had some degree of prolapse before surgery. Currently, surgical techniques using synthetic materials fixed close to the spine help reduce this risk. Therefore, in summary, women who undergo hysterectomy for benign conditions without prolapse generally do not have an increased risk of bladder prolapse.
