WHAT IS CLOMIPRAMINE? Clomipramine is a tricyclic antidepressant (TCA) belonging to the dibenzazepine group. It is unique among all TCAs because it has the most potent serotonin reuptake inhibition of any drug in its class - stronger than most SSRIs. It was first synthesized in 1964 and remains on the WHO List of Essential Medicines to this day. MECHANISM OF ACTION Clomipramine primarily inhibits the reuptake of serotonin (5-HT) and, to a lesser extent, norepinephrine. Its active metabolite, desmethylclomipramine, has significant noradrenergic activity. It also blocks histamine H1 receptors (causing sedation), muscarinic acetylcholine receptors (causing anticholinergic effects), and alpha-1 adrenergic receptors (causing orthostatic hypotension). INDICATIONS (USES) Approved Use: Obsessive Compulsive Disorder (OCD) - adults and children above 10 years This is its primary and most evidence-based indication Off-Label Uses: Major Depressive Disorder Panic Disorder Social Anxiety Disorder Phobias Chronic pain syndromes Cataplexy associated with narcolepsy Premature ejaculation Body Dysmorphic Disorder (BDD) Trichotillomania Insomnia (adjunctive) Treatment-resistant depression (augmentation) ADVANTAGES OVER OTHER DRUGS 1.
