Complex Clear Aligner Treatment Case | High-Angle & Severe Crowding

Challenging clear aligner case: skeletal high-angle pattern with severe full-arch crowding and congenital agenesis of tooth 32. The entire treatment plan was digitally engineered based on precise CBCT 3D assessment for safer, more predictable invisible orthodontic results. To resolve massive arch deficiency and achieve stable vertical & occlusal control, a strategic multi-tooth extraction protocol was performed: 18, 28, 48, 15, 24, 25, 35, 44. Extracting impacted third molars eliminates inflammation risks and crowding relapse. Selective premolar/posterior tooth extraction provides sufficient controlled space for full-arch alignment and decompression in this high-angle case. Anterior IPR was applied for fine torque adjustment and better anterior esthetics. Customized dentition compensation and occlusal finishing were designed to accommodate the congenital missing of 32.
