Second, the sides of the fingers. Third, the abdomen (especially the hips) and the armpits. And fourth, the buttocks, thighs, and genital area. Scabies lesions appear in these areas, as explained in the previous video on this topic, which we'll leave here for you to watch; we recommend it. The application of these medications, these topical drugs, whether creams or lotions, should be done on the entire body from the neck down, and in young children under two years old, this includes the head. I always say, generally, that we should try not to leave any healthy or seemingly healthy skin untreated when applying these medications. Now, what are the specific medications we can use? Because up until now, Lori, we've been mentioning many general measures, but the moment you may have been waiting for has arrived: we're going to mention the medications we can specifically use to treat scabies. First, there are permethrins, 5% creams, which are currently considered the best therapeutic option for children and adolescents. Second, we can mention lindane, which we're only mentioning because it's available on the market, but it's not our preferred option because it's not recommended due to its high neurotoxicity; that is, it affects the central nervous system, and therefore we prefer not to use it. And third, we can mention sulfur-containing petroleum jelly; In our setting, particularly during this scabies crisis and epidemic we've been facing, we've used it with very good results, and it's the treatment of choice for pregnant women, breastfeeding mothers, and newborns. Consider how effective sulfur-based petroleum jelly has been: studies have shown an efficacy of 71%. Finally, there's benzyl benzoate, which can be prescribed, but with caution, as it causes irritation in sensitive skin and when it comes into contact with mucous membranes. That's why we prefer not to use it in young children. But there are also oral treatments, such as ivermectin, which has shown excellent results. I want you to know that it's a very safe and effective treatment alternative, and its administration is simpler than topical treatments, of course, because it's oral. Studies have shown—and this is interesting—that ivermectin and topical permethrin have similar effectiveness against the mite. However, ivermectin treatment has the advantage of being easier to administer, requiring only a single dose that is repeated the following week—just look how easy and convenient it is! Therefore, the chances of treatment failure are minimal. The only drawback, and this is important for Ahmed to know, is that ivermectin should not be given to pregnant women, breastfeeding mothers, or children weighing less than 15 kg. In short, with appropriate specific treatment and adherence to the general measures we have discussed, the chances of treatment failure are extremely small. Then we will have cured scabies in our children. And we have now reached the end of this video. We hope this information has been very useful, and if you don't have anyone in your family with this disease, then share it with friends because perhaps, along with us, they can help treat this baby well and avoid the treatment failures so dreaded by parents and us doctors.
