There are several surgical options. Your doctor will help you decide which one is best for you. In addition to breast surgery, some or all of the lymph nodes under your arm may need to be removed. There are two main surgical procedures for breast cancer: the first is breast-conserving surgery, or lumpectomy, which is always followed by radiation therapy. The second is a mastectomy, which involves the removal of the entire breast. It can be combined with breast reconstruction surgery. In both procedures, some lymph nodes may need to be removed. A lumpectomy preserves the appearance of the breast. The tumor is removed along with a margin of surrounding tissue. In this procedure, the surgeon makes an incision in the skin and removes the tumor with a margin of healthy tissue. This is sent to a pathologist for analysis to ensure that no cancer is present in the surrounding tissue. The wound is then closed with sutures. In a mastectomy, the entire breast is removed. In a simple mastectomy, only the breast tissue is removed. The surgeon begins with an incision to remove an oval of skin that includes the nipple. The breast tissue and underlying muscle are then separated from the skin and removed for analysis. Finally, the surgeon inserts drains to prevent fluid buildup. A modified radical mastectomy is a procedure in which the breast tissue is removed along with some or all of the lymph nodes under the arm. In addition to a lumpectomy or mastectomy, a sentinel lymph node biopsy is often performed. This may be done if a clinical examination or imaging study shows no evidence that the cancer has spread to the lymph nodes. The purpose of a biopsy is to identify and examine the first lymph nodes draining from the breast to ensure that the cancer has not spread to that area. To do this, a dye is injected into the breast to determine which lymph nodes are the sentinel lymph nodes. The surgeon removes one to three sentinel lymph nodes for analysis. If these nodes are cancer-free, it is unlikely that the other lymph nodes will be. If these sentinel lymph nodes are cancerous, the remaining lymph nodes under the arm, called axillary lymph nodes, may also be cancerous. In this case, a complete axillary lymph node dissection may be recommended to remove most of the nodes in this area. An additional option after a mastectomy is breast reconstruction. Breast reconstruction uses implants or tissue from other parts of the body, such as the abdomen or buttocks, to create a new breast. In some cases, the surgeon can also preserve the nipple. Reconstruction can be performed immediately after the mastectomy, known as immediate reconstruction, or at a later date, called delayed reconstruction.
