Gout of the Knee

A 45-year-old athletic man presented with persistent anterior knee pain and gradual swelling of his patellar tendon. Despite physical therapy and NSAIDs, his symptoms worsened over ten years and finally forced him to stop running marathons and triathlons. Imaging revealed a 2 × 2 × 7 cm fusiform intratendinous mass in the patellar tendon, initially thought to be chronic jumper’s knee or tendon degeneration. However, intra-operative findings and histology confirmed a gouty tophus within the tendon—a rare manifestation of Gout.
