Microscopic Hematuria - CKD E-Learning - January 22, 2020
Dr. Peter Birks, a nephrologist at Fraser Health, presents a talk on microscopic hematuria. 0:05:24 – Background He begins by providing background information and definitions of microscopic hematuria. Significant microscopic hematuria is defined as greater than or equal to 3 RBC per high power field on urine microscopy. 0:07:58 – Urine Testing He discusses testing for microscopic hematuria. 0:10:57 – Approaches Dr. Birks gives an overview of systemic and organ-specific causes of the condition, including urinary, bladder, prostate and renal causes. In terms of screening, he notes that urine cytology is no longer recommended. 0:15:08 – Investigation of Microscopic Hematuria He notes that once it is detected, other causes (e.g., menstruation, vigorous exercise) need to be ruled out, and that all patients should received renal imaging. KUB ultrasound is the preferred initial investigation in most cases. He describes what might indicate parenchymal disease, and when a referral to urology for cystoscopy should occur. 0:22:08 – Renal Parenchymal Disease Dr. Birks talks about renal parenchymal diseases such as Glomerulonephritis, Alport syndrome, acute interstitial nephritis (AIN), polycystic kidney disease, sickle cell anemia, kidney stones, and other conditions. 0:42:51 – Summary He provides a summary of his talk and takes questions from the audience. #Hematuria
