In recent years, there has been great progress in neurosurgery for the diagnosis and treatment of brain tumors. In this video, Dr. Iuri Neville discusses brain tumors. What are brain tumors? Brain tumors can be benign, malignant, originate from brain cells, or stem from malignant tumor cells outside the brain (brain metastases). Symptoms can include headaches, motor deficits, seizures, among others. Headache is a very common symptom, and many believe that constant and severe headaches may be associated with a brain tumor. Most headaches are not related to brain tumors; however, there are some warning signs that may link headaches to a brain tumor: 1) changes in the characteristics of the pain, pain upon standing, pain upon exertion, blurred vision, among others, which are explained in this video by Dr. Iuri Neville. It is worth noting that some tumors are not related to headaches, with the first manifestation being motor deficits, seizures, among others. Some brain tumors, when diagnosed, are benign, small, and located in relatively unimportant areas of the brain. Other tumors require surgical removal, the goal of which is to remove as much of the tumor as possible while preserving the patient's function. Some tumors are treated with whole-brain radiotherapy or radiosurgery, a technique that focuses ionizing waves exclusively on the tumor, thus targeting the brain's neurons. Another way to treat brain tumors is through chemotherapy, either local (infusion of chemotherapeutic agents directly into the brain) or systemic through infusion into the bloodstream or tablets. The revolution lies in precision medicine: the tumor and its genetic characteristics are studied, allowing for more specific treatments. Precision medicine is revolutionizing tumor treatment. In the case of surgery, the main objective is to remove as much of the tumor as possible without leaving any sequelae in the patient. First, there is meticulous planning for brain navigation through examinations such as tractography and, in the operating room, neuronavigation. Operating with the patient awake is very important to preserve functions such as language and body movements. Before removing the tumor, the neurosurgeon performs a detailed mapping of the brain's main functions, such as movement and language, to individualize the location of these functions in the patient's brain and increase safety against possible sequelae during tumor resection.
