Everyone knows a little about “breaks.” Your neighbor, your aunt, or someone close to you has had a fracture, used a cast, or even had surgery for it. Yes, this is something that happens frequently and is an eternal field of study in orthopedics. In this post, we will try to clarify a little about fractures of this bone in our knee, a sesamoid bone, which has a fundamental function for the proper functioning of this noble joint. This bone structure is responsible for increasing strength and agility in flexion-extension movements of the knee, functioning as a pulley. A structure with little protection, with only skin and a little fat on top, it is very susceptible to trauma and consequently to fractures, especially in falls and car accidents. A fracture of the patella can occur due to direct trauma, which is the most common in care, but it can occur indirectly, due to a movement that causes excessive contraction of the thigh muscles. What are the symptoms? How is the diagnosis made? A person with a fracture experiences intense pain in the anterior region of the knee, often with local hematoma, and it is common for the patient to be unable to extend the knee, much less walk. After the initial treatment, an X-ray is the best test to be performed. With this, we can confirm the diagnosis and, in most cases, define the treatment, but in some cases, we use a CT scan to better understand the fracture. As always, a fracture can be treated with or without surgery. This is decided by technical criteria, such as the type of fracture and how displaced it is. I will now describe the two most commonly used types of treatment. 1) Conservative treatment (without surgery): used for fractures without displacement or minimally displaced. – We use an immobilizer in extension for 4 to 6 weeks – I release the load (step) according to pain. This means that if the patient has little pain when walking, this is allowed. However, walking should be done with the immobilizer. – We begin isometric thigh contraction exercises to keep the muscles active. – We maintain follow-up with consultations and serial x-rays. After bone consolidation, the knee begins to gain mobility, followed by muscle strength gain and control. All of this with the help of physiotherapy. If properly indicated, the chance of success of this treatment is around 95%. 2) Surgical treatment The vast majority of patella fractures require surgery for good evolution, this is because due to the great traction force that exists from the muscles on this bone, after a fracture it tends to separate, that is, it loses its pulley function, which is why there is a need to reestablish its anatomy with a surgical procedure. Can there be complications? As with any fracture, there are always possible complications. Non-consolidation of the fracture may occur with any treatment, a complication associated with some risk factors such as smoking or advanced age. A new fracture may also occur in the same location, with much less trauma. This usually happens in the first 3 months of treatment. After the surgical procedure, using a tension band, many patients are bothered by the metal material placed on the patella. This may lead to a new surgery to remove this material. However, this is the patient's choice. In the long term, a more complicated fracture may develop into osteoarthritis (wear) in the patella joint. This is a relatively common consequence of all joint fractures. This should be treated individually. But in the end, a well-managed patella fracture leads to very satisfactory results for the patient and the doctor.
