Hello! Let's talk about the treatment of De Quervain's tenosynovitis. In this video, we'll remind you that De Quervain's tenosynovitis causes pain and functional impotence, with loss of strength in the wrist and thumb of the affected hand. This is due to inflammation around the abductor pollicis longus and extensor pollicis brevis, under the extensor retinaculum, in the first extensor compartment of the wrist, in the region of the radial styloid. How do we perform conservative treatment? Continuous use of a forearm-palmar orthosis with thumb blocking for the first few days. Nighttime use of a forearm-palmar orthosis with thumb blocking for a prolonged period. Use of taping to improve proprioception. Stretching. Isometric and concentric strengthening of the involved muscles. Hand therapy, motor physiotherapy, or occupational therapy. - Anti-inflammatory medication: nonsteroidal or depot corticosteroid. If the patient doesn't improve, I recommend one or two wrist injections to improve the condition. If injections, combined with other treatments, don't resolve the symptoms, we may recommend surgical treatment. However, both injections and surgery are topics to be addressed in future videos.
