Well, even if the game isn't very innovative, it is well known for being more difficult than Contra — there are more enemies on the screen at once, and the Konami code only gives you 10 lives instead of 30. The story features the two most heterosexual shirtless guys ever, Mad Dog and Scorpion, in a mission to kill Red Falcon.
