Explaining Root cause analysis using the 5 whys technique for incident investigations Root cause analysis is important in incident investigation. There are different methods to reach the root cause conclusion. In this incident investigation training video I am going to provide an explanation to the 5 whys technique using the forklift example we have been reviewing thus far. The idea is to ask 5, hence the name 5 whys, to determine a root cause for the incident.
