Post-herpetic pain is a complication of herpes zoster, also known as shingles, caused by the reactivation of the varicella-zoster virus, the same virus that causes chickenpox. This virus, present in 95% of the population, remains dormant in the nerves and, when the immune system weakens, reactivates, causing blisters and intense pain, which can be described as a shock, burning, or itching. Advanced age, the intensity of the initial pain, immune system impairment, and the severity/extent of the lesions are risk factors for the chronicity of post-herpetic pain. Although the pain can last 3 to 6 months in 90% of patients, about 10% may develop chronic post-herpetic neuralgia, which can be permanent. In addition to early antiviral treatment, pain management should be initiated as soon as possible. The first-line medications include gabapentinoids (gabapentin and pregabalin) in high doses, and antidepressants, especially tricyclics such as amitriptyline, which is considered one of the best medications for this pain. Dual-agent antidepressants such as duloxetine and venlafaxine are also effective and can be combined. Dual-agent opioids, such as tramadol and tapentadol, are useful early in treatment for immediate pain relief while other medications reach their full effect. Topical treatment of post-herpetic pain, with the skin already intact, can be done with high-concentration lidocaine gel or patches, which can be combined with compounded gabapentin and amitriptyline. Capsaicin cream (the active ingredient in chili peppers) is another topical option for inhibiting neuropathic pain. For persistent cases, nerve root blocks and infiltrations with anesthetics and corticosteroids, guided by ultrasound, are effective procedures. Neuromodulation, such as transcranial magnetic stimulation, has shown promise in modulating neuropathic pain with few side effects. Complementary techniques such as acupuncture and physical therapy also help with analgesia and desensitization. Supplements such as vitamins B12, B complex, and omega-3 can aid neuronal regeneration. Botulinum toxin applied to the painful area can alleviate symptoms, especially allodynia.
