Electroneuromyography - What is the Exam Like? What is the Importance of Electroneuromyography in Diagnosis? The electroneuromyography exam is a highly specialized neurophysiological examination that evaluates the peripheral nervous system and the muscular system. Electroneuromyography is performed by a neurophysiologist, a neurologist specializing in complementary exams for studying the functioning of the nervous system. In this video, Dr. Diego de Castro explains the electroneuromyography exam, its importance in diagnosis, and how it is performed. What Does Electroneuromyography Mean? The term electroneuromyography means: Electro = "electric current" Neuro = "nerve" Myography = "recording of muscle activity". Therefore, the electroneuromyography exam uses electrical stimulation to study nerves and muscles, recording activities and making a mathematical interpretation of these findings. This is important because it helps confirm a suspected alteration in nerve function. How it's Performed When performing an electromyography exam, we use an electrical stimulus, literally a shock, on the nerve and capture the response in the region where the nerve is responsible for movement or sensation. This electrical stimulus certainly generates discomfort because it uses the principle of electrical stimulation, but the exam is very easy to perform. Once the electrical stimulus is finished, the next part of the exam involves inserting a needle into the muscle to assess muscle function: how strong the muscle is, how strong the activation pattern of that muscle is. What is its Importance? When a nerve is compressed, it's possible to document that after applying an electrical stimulus, the response obtained is not the normal response. And more than that: it's possible to grade the severity and intensity of the nerve injury. This is so important, especially in the differential diagnosis between tendinitis and carpal tunnel syndrome, because in carpal tunnel syndrome, the nerve presents an alteration in its function, and we can measure the size of this lesion, the extent of neurological impairment, through electromyography, which also has repercussions on treatment. And electromyography doesn't just study one nerve. With it, we can evaluate the other nerves of the hand, the nerves of the shoulder, and the spine, to identify the cause of symptoms such as numbness in the hand and loss of strength, among others.
