Use of Penile Doppler Ultrasound Testing in the Diagnosis of Erectile Dysfunction | Podcast

Dr. Niyazi Umut Özdemir explains: Imaging modalities used in the diagnosis of vascular (vasculogenic) erectile dysfunction (erectile impairment) aim to evaluate the patient’s anatomical and functional status, guide treatment planning, and particularly assist in surgical decision-making when necessary. Each modality has its own advantages and limitations. The primary imaging method used in this field is Penile Color Doppler Ultrasonography (PCDU). Today, PCDU is considered the gold standard and first-line imaging modality for the evaluation of vasculogenic ED. Advantages: • Functional and Anatomical Assessment: Allows objective and real-time evaluation of penile hemodynamics (blood flow). It differentiates arterial insufficiency (via Peak Systolic Velocity – PSV measurement) from veno-occlusive dysfunction (via End-Diastolic Velocity – EDV measurement). • Minimal Invasiveness and Cost Efficiency: Compared to other methods, it is less invasive, more cost-effective, and widely accessible. • Detection of Additional Pathologies: Provides excellent visualization of penile anatomy and can detect structural abnormalities such as Peyronie’s plaques, fibrosis, tumors, or penile fracture. • Predicting Treatment Response: Plays a critical role in evaluating patients who do not respond to oral medications (PDE5 inhibitors). Limitations: • Operator Dependency: The accuracy of the test largely depends on the experience of the operator and the quality of the ultrasound equipment. • Psychological Factors and Anxiety: Anxiety or fear of injection can trigger sympathetic discharge, suppressing arterial flow. This may lead to “false venous leak” or misdiagnosis of arterial insufficiency, especially in younger patients. • Invasiveness and Complications: Requires intracavernosal injection.
