Hypospadias is a congenital anomaly that must be repaired in childhood. Hypospadias is a condition in which the urethra, which normally opens at the tip of the penis, opens anywhere on the underside of the penis, from the perineum to the tip of the penis. It occurs in one in 200 male births. It is more common in children born through in vitro fertilization. Hypospadias is classified into three categories: anterior, middle, and posterior. Diagnosis of Hypospadias Diagnosis is made immediately after birth with a simple physical examination. The foreskin is underdeveloped on the underside of the penis (prophetic circumcision). It is hood-shaped. The urethra is lower than normal. The glans penis may be separated at the base. There is a curvature during erection. Parents should take side-view photographs of the child while he or she is asleep and when the penis is erect to determine the degree of curvature. This will help the doctor make a diagnosis. The degree of curvature can cause problems with sperm transfer and may also lead to infertility. Should Children With Hypospadias Be Circumcised? Children should never be circumcised, as circumcision is necessary for a full-fledged surgery. Hypospadias Surgery The general rule, especially for complex surgeries, is that the first surgery is the most likely to be successful because the tissue integrity is intact. A failed hypospadias surgery due to poor surgery may not yield satisfactory results despite numerous subsequent surgeries. However, success can be achieved with experienced surgeons. No two surgeries performed on patients with hypospadias (who have previously had unsuccessful hypospadias surgery) are alike. Each case must be evaluated individually. A specialist surgeon must be knowledgeable about all types of surgery, have extensive knowledge of tissue transfer techniques, and possess extensive experience. There is no medical treatment for hypospadias. In cases where there is no curvature and the opening is 1-2 mm below normal, surgery may not be necessary. This is because the patient is urinating normally and has no problems with erectile function. Hypospadias surgery is generally performed between one and two years of age. It is performed under general anesthesia. Patients can be discharged within a day. The goal is to correct the curvature of the penis, restore the urethra to its normal position, and create a smooth and satisfactory structure that facilitates sexual intercourse and sperm transfer. One-stage procedures can yield successful results in anterior and mid-segment hypospadias surgeries. Two-stage, or two-stage, surgical procedures are more successful in advanced hypospadias. Attempting to correct advanced hypospadias in a single stage is a mistake. It often yields unsuccessful results. What is a Hypospadias Injury? Patients who have previously undergone unsuccessful surgery are called hypospadias injures. If the urethra cannot be created and the penile curvature cannot be corrected as a result of these unsuccessful surgeries, there is no tissue left to use around the genitals. These patients are defined as hypospadias injures. Patients with hypospadias injures often present in older ages because they have previously undergone numerous surgeries and their penises have not been straightened. Only physicians with much more experience and tissue transfer expertise can perform the treatment.
