Myasthenia gravis is a neuromuscular disorder caused by a defect in the transmission of signals between nerves and muscles, an interruption of normal communication between nerves and muscles. Myasthenia gravis is part of the autoimmune disease group, which is related to thyroid diseases, type 1 diabetes, multiple sclerosis, rheumatoid arthritis, and lupus erythematosus. Wait, don't get me wrong: it doesn't mean that everyone with myasthenia gravis has all of these diseases! I just said that it is, like them, an autoimmune disease. (Just a rare one.) BUT (because there's a damn "but"), people with myasthenia gravis may be more prone to other autoimmune diseases, and I'm referring to rheumatoid arthritis and lupus. Who can myasthenia gravis affect? Absolutely anyone, whether they're a child or an adult, a woman or a man. However, if we look at the age at which the disease begins, there are two peaks of incidence: in women between the ages of 20 and 40 (and it is called early-onset myasthenia) and in men over 50 (in which case it is called late-onset myasthenia). The disease is not inherited, BUT (again "but"!), a very small percentage of myasthenic people (2%) inherit some neurotransmission defects, in their case it is congenital myasthenia. Only in their case we are NOT dealing with an autoimmune disease, because the immune system is NOT involved. What happens in the body of a person suffering from myasthenia gravis? All the muscles in the body are controlled by nerve impulses that originate in the brain. The impulses travel along the nerves to the place where the nerves meet the muscle fibers. But they do not immediately connect with each other, because there, between the nerve ending and the muscle fiber, there is a space called the neuromuscular junction. When the nerve impulse from the brain reaches the nerve ending, it releases a substance called acetylcholine. The substance travels through that space to the muscle fiber and attaches to several receptor sites, and the muscle contracts only when enough sites have been activated by acetylcholine. This happens normally. In people with myasthenia gravis, the problem is that antibodies "kill" about 80% of the receptor sites (and destroy them faster than the body can replace them) and then... no more neurotransmission. Do you understand? Okay... How does myasthenia gravis manifest itself. That is, what symptoms does it have. (Now you seem to be starting to be more interested, right?!) I'll list them for you, but have a little patience because you're going to hear from a patient herself, Adriana Harja, what she feels, who actually has the disease. The main symptom of the disease is weakness of voluntary muscles, a weakness that increases as the muscle is used, but improves with rest. In most patients with myasthenia gravis, the first muscles affected are the eye muscles, causing drooping of one or both eyelids or double vision. Others experience weakness in the muscles of the face, neck, trunk, or limbs. In severe cases, breathing can be so difficult that it is necessary to be connected to an assisted breathing machine. When the facial muscles are affected, the patient has difficulty swallowing, chewing food becomes difficult or even impossible, and may also experience changes in facial expression.
