"Isotopic or radionuclide therapy is a type of oncological treatment that consists of administering a radioactive substance, with high energy, which has a certain affinity for a certain tumor tissue. Usually, the radioactive isotope is bound to a molecule and that molecule binds to the tumor cell. This does not happen in the case of radioactive iodine, which has physiological tropism for the thyroid cell. Not every type of thyroid cancer benefits from radioactive iodine treatment. Isotopic therapy is applied in differentiated thyroid cancers, which represent 80 percent of the total number of thyroid cancers. Isotopic therapy is performed in patients with differentiated thyroid cancer who have already undergone surgery. It is always performed only after total thyroidectomy, that is, removal of the thyroid gland. We cannot treat the patient with thyroid cancer with the thyroid gland in place, because we would have some major adverse effects. Therefore, patients will undergo isotopic therapy after the intervention surgical, having the correct diagnosis and staging of thyroid cancer. The purpose of this treatment is to ablate any remnants of tumor tissue or normal thyroid tissue, which most often remains after total thyroidectomy. Behind the thyroid gland are the parathyroid glands, which play an essential role in calcium metabolism. And the endocrinologist surgeon tries to preserve these parathyroid glands. For this reason, a small remnant of normal thyroid tissue remains, which the radioactive iodine treatment intends to destroy. This is to more easily monitor the patient in the long term and to have a greater sensitivity of the tumor marker, thyroglobulin. This can be increased not only as a result of the persistence of tumor tissue, but also as a result of the persistence of normal thyroid glandular tissue," said doctor Mirela Gherghe in the video interview.
