Long Term Chronic UTI Treatment: Chronic UTI Treatment, with Dr. Stewart Bundrick, Part 2

Because chronic or persistent UTI is difficult to treat, many clinicians are opting for longer term approaches. In this video, Dr. Stewart Bundrick addresses treatment options for chronic UTI, and his preferred method - long term antibiotic treatment. ● For chronic UTI, a thorough patient history must be completed. Patients are asked about the frequency and urgency of urination and whether they’re experiencing pain in their lower abdomen or urethra. ● Follow up consultations will require the same questions being answered. ● For chronic UTI, Dr. Bundrick typically employs long-term full dose antibiotic treatment. ● A duration of one year’s treatment is typical for most patients though, if the patient is feeling well, they may discontinue treatment after three months in order to assess. ● Success of this approach is affected by a patient’s age and the length of time they have suffered the condition. ● Long-term treatment is required because the infection is present deep in the lining of the bladder wall. It takes time for the infection to be eradicated and for the bladder to reestablish itself. ● If a patient does not improve after three months of treatment, he will try a different antibiotic treatment. ● Dr. Bundrick believes that long-term antibiotic treatment is a safe method of treatment. ● To balance an antibiotic’s effect on gut flora, Dr. Bundrick recommends using a probiotic. ● In the minority of cases where patients cannot tolerate antibiotics or have suffered from Clostridium difficile, Dr. Bundrick recommends an antibiotic infusion into the urinary tract. ● In the unlikely event that long-term antibiotic use fails, Dr. Bundrick’s next course of action would be IV treatment.
