How hard is the extreme game mode in Mini Metro?

I played St. Petersburg in Mini Metro on extreme mode, where you can't change your lines or move your trains once the game starts. That means every decision matters from the very beginning, and there's no fixing mistakes later. I ended up landing in the Top 25%.
