This is often related to conditions such as pulmonary edema, where fluid builds up in the lung tissue and air sacs, or pleural effusion, where fluid collects between the lungs and chest wall. Both conditions can interfere with normal breathing and make it harder to rest comfortably. Symptoms often worsen when lying flat because body position changes how fluid is distributed. When you recline, gravity no longer helps reduce pressure on the lungs, and fluid can shift in a way that limits lung expansion. In pulmonary edema, lying down can also increase blood flow back to the heart and lungs, which may worsen fluid buildup in the alveoli. This can lead to shortness of breath soon after getting into bed. One common symptom is orthopnea, which means difficulty breathing while lying flat. Many people need extra pillows or prefer sleeping in a recliner to breathe more comfortably. In more severe cases, they may wake up suddenly gasping for air, known as paroxysmal nocturnal dyspnea. These breathing problems can disrupt sleep, causing frequent awakenings, fatigue, poor concentration, and reduced quality of life. Elevating the upper body is one of the simplest ways to improve comfort. Medical treatment is also important and depends on the cause. Diuretics may be used for heart failure-related fluid overload, thoracentesis may remove pleural fluid, and oxygen therapy or CPAP/BiPAP may help improve oxygenation in certain patients. Lifestyle adjustments may also help, such as limiting evening fluid intake, reducing salt, and creating a cool, well-ventilated sleep environment. However, worsening shortness of breath, chest pain, persistent coughing, or leg swelling should be evaluated by a healthcare provider.
