Initially, she received positive feedback from those around her saying she had become slimmer and prettier. However, starting from the 6th week, when weight loss began in earnest, the reactions from those around her changed. So I went to a dermatologist, and they told me that Ulthermage would provide a lifting effect, so I got it twice. However, every time I get it, it feels like my face has actually sunken in more. Am I imagining this? This is the question the patient sent. Before weight-loss drugs became available, there was a method called bariatric surgery, also known as gastric banding, which reduces the size of the stomach to induce a feeling of fullness even with small amounts of food, thereby facilitating weight loss. While becoming slim is desirable, the rapid weight loss often led to skin sagging. As a result, it is common in the United States to undergo surgeries known as 'Body Contouring Surgery' or 'Body Lift' to remove excess skin and lift the skin. A particularly famous incident involved Demi Moore, who was over sixty, and her elegant legs. There were even articles suggesting knee lifts, asking, "How can someone over sixty have such beautiful legs?" In the case of today's patient, receiving Wigobi Ozempic injections helped improve their figure while wearing skinny jeans. Regarding the face, the sagging skin under the chin tightened, making the face appear slimmer. As further weight is lost, the platysma muscle under the chin begins to become exposed. This leads to a distinct "bone exposure" phenomenon—where the orbital bone becomes visible through the skin surface—resulting in a face that looks even older. A face where the brow bone, orbital bone, and cheekbone become visible due to rapid weight loss is called an "Ozempic Face." In the past, this condition frequently occurred after bariatric surgeries such as gastrectomy. However, with the emergence of highly effective weight-loss medications like Saxenda, Wigobi, and Maunzaro, and despite advancements in gastric banding techniques to endoscopic and robotic methods, the number of such surgeries is reported to have decreased by more than half. An Ozempic Face involves not only the phenomenon of facial bone blunting but also changes in the quality of the skin itself—specifically, the thickness and elasticity of the dermis. To explain this step-by-step, this is because the skin does not return to its original state (step 1) simply because it stretches when weight increases (step 2) and then returns to step 1 when weight is lost. To use a balloon analogy, if you inflate a balloon, leave it for a month or two, and then let the air out, it doesn't return to its original elastic state. Instead, as shown on the right, the air escapes while the skin surface area has expanded, causing the face to sag. Just as you cannot simply lift and reattach sagging tissue without removing the excess skin when losing belly fat—as shown in the example on the bottom left—this means that a lifting effect cannot be achieved on the face without removing the excess skin. While it is good that weight has decreased after injecting Ozempic, Wigobi, or Maunzaro—diet drugs that are currently enjoying sensational popularity—an increasing number of patients, particularly middle-aged and older women whose skin elasticity has begun to decline, report that they look much older.
