All 18 people on board were killed three miles short of the runway. Flight 5719 was a routine commuter run into northern Minnesota, but the conditions that night were anything but routine — a 400-foot overcast, freezing drizzle, light snow, and mixed icing in the clouds. The crew briefed the localizer back course approach to Runway 13, a non-precision procedure with no glideslope and no autopilot to fly it. The captain delayed the descent, apparently to limit time in the icing layer. By the time he initiated it, the aircraft was roughly 1,200 feet above where it should have been at the final approach fix. What followed was an aggressive slam-dunk — descent rates climbing to 3,000 feet per minute, the approach completely unstabilized, and altitude awareness slipping away inside night IMC. The first officer had concerns. The cockpit voice recorder captured a captain whose intimidation and authoritarian style had been flagged before, and a coordination breakdown that left no one effectively cross-checking the descent. The aircraft struck a wooded hillside still well above field elevation. The NTSB ultimately attributed the crash to the captain's actions causing a breakdown in crew coordination and loss of altitude awareness during an unstabilized approach in night IMC, with contributing factors including the company's failure to address previously identified deficiencies in the captain's airmanship and CRM, an unapproved approach practice that had spread through the operation, and inadequate FAA surveillance of the carrier.
