Effects and side effects of antidepressants for pain therapy, especially for conditions such as migraines, fibromyalgia, chronic (joint) pain, and nerve pain (so-called neuropathic pain). In chronic pain, so-called neurochemical changes are often found in the pain-processing areas, i.e., the brain and spinal cord. People with chronic pain have an increased need for the neurotransmitters serotonin and norepinephrine. These influence both pain perception and sleep and mood. If the pain persists for years, the neurotransmitter reserves become exhausted, and the pain filters in the brain become increasingly permeable. The result: The smallest impulse is enough to trigger severe pain. Every stimulus is then transmitted unfiltered to the brain, and the affected person perceives even the smallest signals as extremely painful. For this reason, antidepressants are very important medications not only for the primary treatment of depression but also as part of pain therapy. Antidepressants are frequently used, especially for so-called neuropathic pain. In this context, in addition to the underlying and concomitant disease (pain and possibly depression), the kinetics and galenics of the active ingredient are also important. For example, the antidepressant amitriptyline reaches its maximum plasma level after approximately four hours. Therefore, it is recommended to take this medication around 6 p.m. as part of pain treatment, as the affected patient otherwise often experiences so-called daytime sleepiness. Typical side effects are: (1) nausea, (2) headaches, (3) weight gain, and (4) fatigue. Tips for nausea and loss of appetite 1. Take the tablets with meals. 2. Eat smaller, more frequent meals. 3. Drink plenty of water (still water). 4. If necessary, take stomach protection medication. Tips for fatigue and exhaustion: 1. Take walks in the fresh air (and in bright light). 2. Take the medication 1-2 hours before bedtime. 3. If depressed, use light therapy during the day. 4. Be careful when driving! Tips to prevent weight gain: 1. Conscious eating: fruits, vegetables, and whole foods. 2. Reduce highly sweetened fruit juices and sweets. 3. Exercise and sport. 4.
