Tibial Tubercle Osteotomy with Arthrex® T3 AMZ System
In this video, we demonstrate tibial tubercle osteotomy (TTO) using the Arthrex T3 AMZ system, a powerful surgical technique designed to correct patellar mal-tracking and relieve anterior knee pain. The tibial tubercle is the bony attachment site of the patellar tendon. When malalignment occurs, the patella may track abnormally within the joint, increasing pressure on the cartilage and leading to pain, instability, and degeneration. 🔧 Procedure Overview The goal of this procedure is anteromedialization (AMZ), shifting the tibial tubercle inward (medially) and forward (anteriorly) to realign the extensor mechanism and offload stress from the patellofemoral joint. The surgeon begins by placing a guide over the tibial tubercle and inserting a reference pin to determine the orientation of the osteotomy cut. A cutting guide is then secured to the tibia, and protective instrumentation is used to shield surrounding soft tissues and neurovascular structures. Using a surgical saw, the osteotomy cut is made while preserving a bone bridge, which acts as a hinge to maintain stability during repositioning. The tibial tubercle fragment is then carefully shifted medially using the hinge mechanism.
