We talk about how to classify decisions by what a wrong one costs, how to set the line between "AI decides" and "AI recommends, human decides," and how to keep that line from sliding just because the AI has been right lately. At Cardinal Trust Financial, the team let AI auto-approve low-value license reassignments — sensible, and it saved hours. Trust crept, and within a quarter it was auto-approving reassignments that touched a restricted, license-controlled application, until an auditor asked who authorized them and the answer was "the system did." The decisions were individually small and collectively a compliance gap. At the Federal Asset Modernization Office (FAMO), the lead deliberately drew the line the other way — AI recommends, a human signs — because in a federal context an unauthorized automated decision isn't a convenience, it's an accountability failure with a name attached.
