Despite therapeutic progress in prostate cancer, treatment resistance remains a major challenge. The INSIDE study aims to better understand DNA damage repair (DDR) alterations beyond BRCA and to generate patient-derived organoids that can be used for genomic and pharmacological profiling. 🧠 Key insights: The INSIDE study is a prospective trial focused on high-risk advanced prostate cancer and DDR gene mutations. As of March 2025, 68 patients had been enrolled in the study. The team developed a pipeline to generate patient-derived organoids (PC-PDOs) from tumor tissue. The study achieved a 77% organoid derivation success rate. 18% of organoids were suitable for pharmacological screening, highlighting the translational value of the model. Genomic profiling used whole-exome sequencing and TSO500-HRD to assess genomic variability and genomic instability scores. Preliminary results suggest DNA-PK, ATM, and ATR as promising pharmacological targets. The findings also suggest potential PARP inhibitor sensitivity beyond BRCA mutations, including selected homologous recombination gene variants. In the presentation, Dr. Arena also highlighted the practical feasibility of building a larger organoid biobank and using these models for molecular and drug-response profiling.
