Self-Direction Services Explained | NYS Guide for Families & Providers

What is Self-Direction and how do you get started? This webinar, hosted by Jenny Young from LIFEPlan CCO and Advance Care Alliance of New York (ACANY), provides a clear overview of Self-Direction services in New York State for members, families, and providers. Self-Direction gives individuals and families more control over their services, including how supports are chosen, managed, and delivered. In this video, you’ll learn: • What Self-Direction is and how it works • The roles of each member in the “Circle of Support” • The steps to start and launch Self-Direction services • How budgeting and service planning works The Circle of Support includes: • Person/Family: Leads decision-making, hires staff, and sets goals • Care Manager: Completes assessments, builds the Life Plan, and submits documents • Support Broker: Helps create and manage the budget and services • Fiscal Intermediary (FI): Handles payroll, hiring paperwork, and budget tracking Getting Started with Self-Direction: • Attend an OPWDD information session • Choose a Support Broker and Fiscal Intermediary • Complete onboarding paperwork • Work with your Care Manager to submit your plan Once approved, services can begin through a formal launch process. Important Tips: • Do not pay for services out-of-pocket before approval • Work closely with your team to build your budget • Use tools like the Disability Services NY directory to find providers This session also includes a Q&A covering real-life questions about services, reimbursement, and getting started.
