A Telstra network outage has resulted in a police investigation into the death of an elderly woman in regional South Australia who couldn't contact triple zero during the service disruption. The outage also forced the entire V-Line regional rail network to shut down for 31 hours, stranding thousands of passengers, as the train tracking system relies on Telstra's 4G network. Telstra conducted 639 welfare checks on triple zero callers who couldn't get help during the outage and faces potential fines exceeding $30 million from the consumer watchdog.
