Unstabilized approach and runway excursion - the history of Kingfisher Airlines flight 4124
In 2009, an ATR 72 from Kingfisher Airlines landed at Mumbai Airport and skidded off the runway. Nobody was hurt, but the aircraft was severely damaged. So, why did this happen? Like most accidents, this accident did not happen because of a single factor but was a result of a chain of events that ultimately lead to the outcome. In aviation, we have a concept called Threat & Error Management, TEM. This is a tool to identify threats that can affect the safety of flight. In this accident, there were many threats.
