Full Face Endoscopic Lift

What is a full face endoscopic facelift? It is a procedure designed for patients who want vertical-diagonal lifting of the face, better eye shape, less puffiness under the eyes, and a sharper neck and jawline. The procedure usually starts with the eyelids. Most often, transconjunctival lower blepharoplasty is performed, meaning the lower eyelids are treated from the inside, without an external skin scar. In some cases, upper blepharoplasty is also added. Then the neck is treated through a small incision under the chin. Through this access, deeper neck structures can be improved. This may include partial removal of subplatysmal fat, partial reduction of the digastric muscles when needed, medial platysmaplasty, and thinning and plication of the platysma. In some patients, an additional incision behind the ear is also used to pull the platysma laterally and create a sharper jawline with a better cervical angle. Then comes the endoscopic part. The incisions are small, usually 1.5 to 3 cm, and hidden in the hair without shaving it. If the goal is mainly brow and temple lifting, the dissection is focused on the temporal area, either under the skin or under the fascia in the SMAS plane. The tissues are released from dense attachments and retaining structures that limit movement, and then repositioned. If more correction is needed in the midface, the dissection continues deeper - this is the deep plane technique. The retaining ligaments are released, and the cheek tissues are mobilized together with the temporal fascia. If a true full face correction is needed, the dissection goes deeper, down to the platysma, so the tissues of the face and neck can be repositioned as one unit. After the tissues are moved, they need to be fixed in the new position. This is very important because the repositioned layer needs stability and support. Absorbable or non-absorbable sutures can be used for this fixation. At the end, the small scalp incisions are closed with sutures or staples and are usually removed quickly.
