Let's consider biopsy for category 3 calcifications (microcircular clusters of calcifications). The other day, a breast surgeon asked me, "What do you think about this?" about a category 3 calcification. A stereo-guided biopsy (mammotome biopsy) revealed that it was a fibroadenoma (benign). Hmm. It's true that each one is round, but there is some variation in size, and above all, there are a fair number of them in a small area. If the patient is elderly and has no desire for a biopsy at all (and wishes to be monitored), then I think there's no need to do it (monitoring is fine), but if the patient is young and would likely experience stress from constant criticism and constant worry that it might be cancer, then a biopsy might be a good idea, I replied. The general rule for category 3 is "monitoring (for about 6 months to begin with)." However, breast specialists recommend the best timing for a biopsy based on the patient's characteristics (including age, other illnesses, personality, and needs), while balancing the benefits and drawbacks of a biopsy (particularly with regard to overdiagnosis and overtreatment).
