Ambitious athletes usually do not follow prescribed training and competition breaks after an infection and want to run themselves healthy - unfortunately, this can also fail. Here, a 30-year-old athlete had participated in a competition a few days after a viral infection (flu, covid, EBV, RSV, etc. are the possible causes here). Afterwards, for the first time in his life, he felt extra beats in his heart rhythm and physical weakness. The ECG showed elevations, which are very rare in young people and in any case suspicious. It came to the suspicion of myocarditis, which can occur if the patient is stressed too soon after an infection. Myocardial infarction would present similarly, but the known risk factors were absent. Heart muscle inflammation (myocarditis) can be well visualized on cardiac MRI. This procedure is the most sensitive imaging modality. In a good 7 minutes, PD Dr. Henning Steen gives an entertaining presentation of how this is done and how reliable statements can be made in mapping without contrast medium and then, if necessary, also with contrast medium. He refers to the necessity that the body should also be supplied with the necessary micronutrients for its performance in order to reduce the risk of damage on the one hand and to improve regeneration on the other. In the Point of Care measurement, this value is available within 30 minutes. Thus, it can also be measured before and after exercise to see if the load was still within limits. The 3-D vector ECG examination at rest and after a defined amount of physical stress, accompanied by heart-specific laboratory parameters, can thus be done before MRI examination in an uncomplicated manner and also more frequently.
