Landmarks start at the ischial tuberosity and linea aspera; we open fascia lata to reveal the hamstrings and posterior femoral cutaneous nerve. The hamstring group includes biceps femoris long head, semitendinosus, and semimembranosus arising from the ischial tuberosity; the biceps femoris short head arises from the linea aspera and is not a true hamstring. Actions are hip extension and knee flexion with tibial rotation medially by semitendinosus and semimembranosus and laterally by biceps femoris. Innervation is sciatic nerve L4 to S3 with tibial division to all true hamstrings and common fibular division to the biceps femoris short head. Vascular supply is via perforating branches of profunda femoris with anastomoses to the popliteal network. We map the popliteal fossa boundaries superolateral biceps femoris, superomedial semimembranosus and semitendinosus, inferolateral and inferomedial gastrocnemius heads and contents tibial nerve most superficial, then popliteal vein, then popliteal artery deepest plus the termination of the small saphenous vein and common fibular nerve coursing along the biceps tendon to the fibular neck. High-yield cadaveric cues include the semimembranosus expansion forming the oblique popliteal ligament, the adductor hiatus where femoral artery becomes popliteal, and the sciatic bifurcation typically at or just above the fossa. Clinical anchors: hamstring strain or proximal avulsion at the ischial tuberosity, tenderness along the ischial origin in sprinters, foot drop risk with common fibular nerve injury at the fibular neck, popliteal aneurysm or Baker cyst compressing neurovascular structures, and safe IM injections placed in the superolateral gluteal quadrant to avoid the sciatic nerve. Imaging pearls cover MRI of hamstring tears and ultrasound guidance for popliteal blocks.
