Therefore, many patients who present with small nodules or bumps on their hands have a family history of a father, uncle, or first-degree relative who presents the same symptoms. This is a possible indication of palmar fibromatosis. Another characteristic of the disease is that they are usually small nodules, which initially cause no discomfort or pain. Therefore, patients often notice them but leave them for observation, not immediately investigating the cause. The problem is that fibromatosis often causes retraction and limited mobility when not treated properly. 01:22 Diagnosis of Palmar Fibromatosis It is always recommended to seek medical attention for an in-person evaluation if you notice any nodule or any unusual condition in your hand to resolve the doubts about whether it is palmar fibromatosis or another problem. If we suspect this, an excellent initial tool for diagnosis is often an ultrasound. A well-performed ultrasound performed by a physician specializing in musculoskeletal medicine can often identify the problem and, more importantly, measure it. Diagnosis is often only the first step, as we need to monitor the progression of the problem to determine what should be done. 02:16 Treatment Criteria After the diagnosis is made, we often observe, depending on the size of the problem—that is, whether the lesion is large, whether it causes mobility restriction. From there, we stratify and understand where the problem is going, and what the best course of action is for this patient. One measure we often use is how much the problem is restricting the ability to extend the fingers, which is one of the main characteristics of fibromatosis. So, when we assess whether or not this patient needs to take action, we assess whether there is a loss of extension capacity, which we generally talk about as around 30º, which is roughly the measurement we use. Based on the assessment, if the hand has this deformity, if it's already established, we may need to opt for some surgical correction. Unfortunately, in Brazil today, we have few resources, so if there's damage, a deformity already present, and movement difficulties, surgery is often the recommended treatment.
