Invasive mechanical ventilation (IMV) is frequently required in pediatric intensive care units (PICUs) to assist gas exchange in critically ill children. While often life-saving, prolonged IMV is associated with increased risks of ventilator-associated pneumonia, ventilator-induced lung injury, and longer PICU and hospital stays, factors that can contribute to increased morbidity and mortality. The ventilator weaning phase—the patient's transition from full ventilatory support to spontaneous breathing—can represent a substantial portion of the total IMV duration, with estimates suggesting that 25% to 50% of mechanical ventilation time is spent in this phase. Therefore, an optimized weaning process is crucial to minimize IMV duration, reduce the risk of extubation failure and reintubation, and ultimately improve clinical outcomes.
